Raffle Contest Rules

Eligibility. The prize drawing is open only to those who [attend] this event and who are [18] years of age as of the date of entry. The drawing is only open to legal residents of [state]. Everence employees and their immediate family members are not eligible to participate in the drawing.

Agreement to Rules. By participating in the drawing, you agree to be fully unconditionally bound by these rules, you represent that you meet the eligibility requirements, and you accept Everence's decisions as final and binding as related to the drawing.

Start and End Dates. Entries will be accepted from [start date and time] through [end date and time]. 

How to Enter. To enter the drawing, you must enter through the [digital form] available via QR code or official paper registration. Entries that are incomplete or do not adhere to the rules may be disqualified. You may enter only once and may not enter more times using multiple email addresses, identities or devices in an attempt to circumvent the rules. If you use fraudulent methods or otherwise attempt to circumvent the rules your submission may be removed from eligibility.

Prizes. Winners will receive [identify prizes and their value]. No cash or other prize substitution permitted except at Everence’s discretion. The prize is non-transferable. Winners will be responsible for any prize-related expenses, including any federal, state, and local taxes. Acceptance of the prize constitutes permission for Everence to use the winner’s name, likeness, and image for purposes of advertising and trade without further compensation, unless prohibited by law.

Odds. The odds of winning depend on the number of eligible entries received.

Winner Selection and Notification. Winners will be selected in a random drawing under Everence’s supervision and will be notified via email within [7 days] following selection. Everence will not be liable for a winner’s failure to receive notices due to email security settings, providing incorrect or non-functioning contact information, or the winner’s failure to check mail. If a winner cannot be contacted, is ineligible, or fails to claim the prize within [7 days] from the notification being sent, the prize may be forfeited, and an alternate winner selected.

Privacy Policy. Information submitted with an entry is subject to the relevant portions of Everence’s Privacy Statement found on its website at https://www.everence.com/resources/corporate/privacy-policy.
